ATSICHS Brisbane is a member of the Institute for Urban Indigenous Health (IUIH).
**Description**:
This position provides gender specific services to be delivered by a person of the same gender. Therefore, it is a genuine occupational requirement that the incumbent be of the same gender as required for services to be delivered under the section 25 of the Anti-Discrimination Act 1991.

About The Role
As a key member of the ATSICHS Brisbane team, The Community Connector is responsible for providing culturally safe and person-centered support to Aboriginal and Torres Strait Islander people, including those with disabilities, Elders, and individuals accessing ATSICHS Brisbane services. The role facilitates access to ATSICHS services, promotes social and emotional wellbeing, and ensures the effective coordination of care across multiple programs, including Social & Emotional Wellbeing (SEWB) Groups, ATSICHS Primary Health Care team and NDIS Outreach and Community Access Groups.
The Community Connector works collaboratively with community, families, and stakeholders to assess needs, provide referrals, and provide support to ATSICHS Brisbane clients.
Duties
Conduct client intake, assessments, and develop individualised care plans based on a strengths-based and culturally responsive framework.
Facilitate warm referrals to internal and external service providers as needed.
Facilitate group programs, workshops, and community outreach initiatives to promote social inclusion and emotional wellbeing.
Engage and empower clients to participate in community activities, cultural programs, and social opportunities.
Monitor and evaluate client progress, adjusting care plans as necessary.
Participate in training, supervision, and professional development opportunities to enhance service delivery
Develop positive working relationships with service providers
Ensure services provided align with the needs of the Aboriginal and Torres Strait Islander community.

Base salary of $80,322 per annum (based on qualifications and experience).
Additional 11.5% Super + access to salary packaging (Increases your take home pay).

**Skills and Experiences**:
Required
Qualification or enrolment in a Certificate, Diploma, or Degree in Social Work, Psychology, Community Services, Disability, Allied Health, or a related field.
Demonstrated ability to provide culturally responsive and person-centered support.
Excellent communication and interpersonal skills.
Ability to work autonomously and as part of a multidisciplinary team.
Strong case management, advocacy, and referral skills.
Working with Children's Blue Card and Working with Disability NDIS Yellow Card or ability to apply.
National Police Certificate.
Driving Licence.
Travel between work sites on a regular basis.
Travel to work in the community on a regular basis.
Preferred
Understanding of the CHSP, NDIS, QCSS, and community support services.
Experience in care coordination, case management, or program facilitation.
Experience facilitating group programs and workshops.
Knowledge of trauma-informed and strengths-based practices.
Flexibility in working hours shall be required, including weekends and evenings.
