**Role Responsibilities**: As a Support Worker, you will be responsible for assisting individuals with a range of needs, from personal care and daily living tasks to promoting independence, social inclusion, and community participation. You will work closely with clients to develop personalized support plans and goals, supporting their physical, emotional, and social well-being in line with their preferences and abilities.

**Key Responsibilities**:

- Provide high-quality personal care and support to individuals with varying disabilities.
- Assist with daily activities such as meal preparation, mobility, personal hygiene, and social participation.
- Work collaboratively with clients and multidisciplinary teams to develop and implement individualized support plans.
- Empower clients to build life skills, increase independence, and enhance their social and community connections.
- Promote and maintain a safe, respectful, and inclusive environment at all times.
- Maintain accurate and up-to-date records, including progress notes and support plans.
- Participate in ongoing training and professional development to stay informed of best practices in disability support.

**Key Requirements**:

- Previous experience in providing support to individuals with disabilities is highly desirable.
- Strong communication skills, with the ability to engage effectively with clients, families, and other professionals.
- A commitment to delivering person-centered, inclusive, and respectful care.
- Empathy, patience, and the ability to work in a dynamic and rewarding environment.
- Flexibility and adaptability to meet the unique needs of each individual.
- Ability to work both independently and as part of a team.
- Relevant qualifications in disability support, health care, or social services (or willingness to obtain).
- First Aid and CPR certification (or willingness to obtain).
