**The Department**

The Mobile Assessment and Treatment Service (MATS), is seeking a passionate and dynamic Registered Nurse to join our highly skilled team.

**The Role**

Your role will primarily be responsible for Intake and Triage of all new referrals coming into the MATS program, clinical coordination of current MATS clients, and delegating the outreach resources they require. You will also be required to provide some direct care in the community at times.

You will have a keen interest in aged care nursing and those with acute and chronic illness. The role requires that you provide leadership for a multidisciplinary team, whilst also be capable of working independently in the community at times. You must be agile and dynamic in order to meet the pace of the service.

The role offers diversity and opportunity to be innovative. We work collaboratively with community stakeholders such as, GP’s, residential care staff, Ambulance Victoria, VVED and other community services, and various specialties across Alfred Health. This allows us to manage our patients in the community thus preventing avoidable transfers and admissions to hospital.

**Qualifications & Experience Required**

Experience in some or all of the following nursing specialities is desired: General Medical, Emergency, Sub-acute or acute Aged Care Nursing specialities.

We are looking for the following key professional attributes and clinical skills:

- knowledge and experience in clinical triage
- experience in the acute care of older persons
- excellent clinical assessment, identification of deterioration, and escalation skills.
- I.V. cannulation; Phlebotomy; Male, female and supra pubic catheterisation; Wound care knowledge and interest; Enteral feeding tube management.
- a commitment to work flexibly with internal and external services
- a high standard of written and verbal communication skills that demonstrates a professional and collegiate attitude.
- A current Victorian Drivers licence is an essential requirement

MATS operate 24 hours a day, 7 days per week.

Shifts are either 0800-1630 or 1130-2000. This role also has an expectation to participate in an after hours "telephone oncall" roster.
