**About the Role**

We are seeking a people focused Team Leader to join our dynamic Community Services and Well-being Division. Leading our team who coordinate the care and services for Home Care Packages (HCP), Commonwealth Home Support Program (CHSP) and Privately funded consumers. Contribute to the improvement of processes and systems to provide the highest quality care to people where and when they need it. This position is Monday to Friday, based at our Moubray Street Prahran office and working from home.

**Key responsibilities are**:
Provide guidance and support to the Team of Care Coordinators and Service Coordinators

Coordinate staffing within consumer needs and preferences and organisational KPIs

Support Royal Freemasons strategic growth plan

**Key Requirements**
- Excellent customer service and attention to detail
- Rostering/Scheduling experience
- Ability to prioritise workload
- Work independently and support the team
- Outstanding problem-solving ability with a ‘can do’ attitude
- Community Aged Care knowledge
- Comfortable learning new computer systems
- MS Office competent

Desirable but not required:

- Second language
- Experience using Healthmetrics eCase PHC
- Experience working during system upgrades, providing end user feedback to the System Administrator
