Disability Service Manager

**Job Summary**:
Verity Care Services is looking for a Disability service manager with an NDIS background. As a service manager, you would be responsible for overseeing and managing the healthcare services provided by Verity Care Services. The Service Manager plays a pivotal role in maintaining the organization's commitment to excellence in healthcare delivery.
**Responsibilities**:
**1. Service Manager**:
- Develop, implement, and evaluate comprehensive care plans for clients, in collaboration with the multidisciplinary team.
- Assess client needs, review medical records, and conduct ongoing evaluations to ensure appropriate care and services.
- Provide clinical expertise and guidance to the care team, ensuring adherence to best practices and standards of care.
- Monitor client progress, identify areas of improvement, and adjust care plans accordingly.
- Act as a liaison between clients, families, and healthcare providers, advocating for the best interests of clients.
**2. Team Leadership and Supervision**:
- Recruit, train, and supervise a team of healthcare professionals, including nurses, caregivers, and support staff.
- Provide ongoing supervision, mentoring, and performance feedback to ensure staff competency and professional growth.
- Foster a collaborative and supportive work environment, promoting effective communication and teamwork.
- Conduct regular team meetings, disseminate relevant information, and facilitate problem-solving and decision-making processes.
**3. Quality Assurance and Compliance**:
- Ensure compliance with regulatory requirements, professional standards, and organizational policies and procedures.
- Implement quality improvement initiatives to enhance the delivery of care and services.
- Monitor and analyze clinical data, identify trends, and recommend strategies for improvement.
- Participate in internal and external audits, addressing any identified deficiencies promptly.
**4. Client and Family Engagement**:
- Establish and maintain positive relationships with clients and their families, promoting open communication and trust.
- Collaborate with clients and families to develop individualized care plans that align with their goals and preferences.
- Provide education and support to clients and families, empowering them to actively participate in their healthcare decisions.
- Resolve client concerns and complaints, ensuring timely and satisfactory resolution.
**Client Acquisition**:
Identify and engage with prospective clients, build relationships, and present Verity Care Services' value proposition to generate new business leads and secure contracts.
**Networking and Representation**:
Represent Verity Care Services at industry events, conferences, and networking forums to enhance the company's visibility and identify potential business leads.
**Partnership Development**:
Identify and cultivate strategic partnerships and collaborative relationships with healthcare providers, hospitals, insurers, community organizations, and other relevant stakeholders.
**5. Collaboration and Coordination**:
- Collaborate with other departments, including administration, nursing, therapy, and social services, to facilitate coordinated care.
- Coordinate and facilitate care transitions, including hospital admissions, discharges, and transfers.
- Act as a liaison with external healthcare providers, community organizations, and regulatory agencies.
**Qualifications**:
Excellent understanding of the NDIS
Relevant qualification or demonstrated equivalent professional experience
A minimum of 2 years of experience is required.
Demonstrated knowledge of NDIS operations
Comprehensive knowledge and understanding of policies and legislation
Approach and willingness to show compassion and care toward participants
Experienced in similar positions
**Salary**: $95,000.00 - $110,000.00 per year
