The Aboriginal Health Practitioner/trainee position within the Assertive Case Management (ACM) program is to work as part of the multi-disciplinary team that provides intensive care coordination to support consumers with chronic and severe substance use issues. The ACM program will target clients with chronic and severe drug and alcohol dependence who experience multiple and complex needs.
