**Description**:
At Halo, we are seeking a dedicated and multi-skilled Support Worker to join our team. As a Support Worker, you will play a crucial role in providing care and support to individuals within our community.
**Key Responsibilities**:

- Assist individuals with their daily living activities, including personal care, meal preparation, and household tasks.
- Provide emotional support and companionship to individuals, promoting their overall well-being.
- Support individuals in accessing community resources and participating in social activities.
- Implement person-centred care plans, ensuring that individual needs and preferences are met.
- Observe and report any changes in an individual's condition or behaviour to the appropriate supervisor.
- Engage in effective communication with individuals, their families, and other healthcare professionals involved in their care.
- Maintain accurate and up-to-date records of care provided and any significant observations.
- Adhere to health and safety guidelines to ensure a safe environment for both the individuals supported and yourself.

**Requirements**:

- Prior experience in a similar support or care role is preferred.
- A compassionate and patient approach towards supporting individuals with diverse needs.
- Excellent communication skills and the ability to work effectively within a team.
- A willingness to undergo relevant background checks as required by regulatory bodies.

**Skills and Experiences**:

- Proven experience in providing support to individuals with diverse needs and abilities

- Proficiency in administering medication and following health and safety protocols
- Strong communication skills, including the ability to listen actively and communicate effectively with individuals, colleagues, and external stakeholders
- Ability to work independently as well as part of a team, demonstrating flexibility and reliability
- A compassionate and empathetic approach towards supporting individuals with their daily living activities
- Experience in implementing support plans, documenting progress, and reporting any concerns or incidents
- Understanding of safeguarding principles and the ability to maintain confidentiality at all times
