**Full Job Description**Capability Care is currently seeking a highly motivated, energetic, and caring Support coordinator to join our team and work alongside our current Support coordinators.As a Support Coordinator you will be responsible to promote empowerment, choice and control and community inclusion of people with disabilities and their families, consistent with their NDIS plan.**About you**We are looking for a sociable and organized Support Coordinator to join the Capability care team. The role involves supporting participants in understanding and implementing funded support in the NDIS plan and linking them to the appropriate services. You will be responsible for establishing and maintaining a positive relationship with the participant and stakeholders.**About the role**Provide Level 1&2 Coordination of Supports.
Conduct risk assessments with the client, based on their NDIS goals and the level of service defined in the NDIA plan.
In conjunction with the client and their family, develop a support plan that clarifies the actions to be taken and outcomes to be achieved, where appropriate.
Conduct assessments of individuals' needs and develop individualized support plans in collaboration with the individual, their family, and other service providers.
Seek new participants.
Coordinate services as defined in the client’s NDIA plan, using a person centered
- based and capacity building approach. This includes informal supports, mainstream services (housing, education, health), specialist services, and NDIS funded supports.
Monitor the use of support coordination hours and update the Manager if additional hours are required.
Assist and empower the client and their family by providing information about the range of mainstream and specialist services, including advocacy services available to them to help support clients in achieving their goals.
- Accept and follow up all referrals for Support Coordination
Maintain complete case notes and gather record of relevant information about other services accessed by client in continuity of support and review and stay up to date on current policies and regulations related to disability services Manage and plan budgets.

**Qualifications**:
Minimum 2 year of Support Coordination experience
Understand NDIS legislation, price guide and rules.
Ability to work independently and part of a team.
Ability to maintain a high degree of confidentiality and manage sensitive information.
Not essential, however highly regarded: relevant territory qualification.
High level of interpersonal skills, including the ability to develop and maintain relationships.
Proactive
Ability to work under pressure.
Excellent written and verbal communication skills
Intermediate in Microsoft Office
Current driver's license.

**Job Types**: Part-time, Casual
Part-time hours: 10 per week

**Salary**: $35.00 - $37.00 per hour
