Dementia Care Navigator

**About the role**
This role supports people living with dementia and their carers in the community to live safely and independently at home for as long as possible. The Dementia Care Navigator provides in-home support, practical tools, and emotional guidance to reduce carer stress and delay the need for hospitalisation or residential aged care. A key part of the role is also building a dementia-aware, inclusive local community and collaborating with local services and teams to ensure holistic, person-centred care.
You’ll work closely with clients, carers, the local Wellbeing Coordinator, and broader community partners, offering education, care navigation, and proactive early interventions to improve quality of life for those affected by dementia.
**This role is offered as a Fixed Term opportunity, Part-Time (up to 0.8 FTE) Contract until June 2026, with possibility of extension. This role is based out of our Daw Park office.**

**Responsibilities**
- Conduct regular wellbeing and cognitive support check-ins with clients in their homes.
- Provide practical tools, memory aids, and environmental modifications to promote autonomy and reduce risk.
- Coordinate access to in-home support services and assistive technologies tailored to cognitive needs.
- Deliver education sessions for clients and families on maintaining independence with dignity.
- Facilitate regular carer support groups and one-on-one emotional wellbeing sessions.
- Connect carers with respite opportunities, including day programs or in-home supports.
- Implement a “carer early warning” system to identify signs of burnout and intervene proactively.
- Provide carers with education on behavioural changes, communication strategies, and community resources.
- Collaborate with the Wellbeing Coordinator to shift time and resources toward proactive engagement (e.g., social prescribing, group wellness sessions, walking groups).
- Develop and co-deliver structured wellbeing activities that encourage physical, emotional, and cognitive health.
- Assist with needs assessments, service navigation, and follow-up to ensure long-term wellbeing plans are in place.
- Monitor and respond to health and behavioural changes to prevent crises before they escalate.
- Coordinate multidisciplinary case conferences to manage complex needs in the community.
- Provide rapid-response support or escalation pathways for carers and clients in distress.
- Build partnerships with local health services (GPs, hospitals) to create continuity of care and better handover processes.
- Conduct training and awareness sessions for local services, and community groups to improve dementia friendliness.
**What do we need from you?**
- Tertiary qualification in Nursing, Allied Health, or a health-related field, with additional training in dementia care.
- Strong understanding of dementia, cognitive changes, and related behaviours.
- Familiarity with environmental design principles that support people living with dementia.
- Experience supporting informal carers through emotional support, education, and burnout prevention strategies.
- Confident facilitating sensitive conversations, including early cognitive decline and future planning.
- Confident in delivering community education or awareness activities to promote dementia-inclusive practices.
- Creative thinking in supporting independence while reducing harm.
