**HammondCare is currently looking for an experienced Clinical Care Advisor to support and inspire the team at Daw Park, SA.**:
The Clinical Care Advisor works within HammondCare’s Mission in Action to provide clinical advice, and hands-on care ensuring the delivery of high-quality, person-centered care that supports clients to live well in their own home. The role supports clinicians to implement evidence-based care, clinical governance processes and continuous quality improvements. The role fosters a culture of excellence and innovation in clinical care through continual professional development and education, utilization of evidence-based current best practice and identification of opportunity for care and service development.

The essence of this role:

- Work alongside and mentor Clinical Partners to ensure delivery of expert clinical care.
- Enhance care delivery and capability by providing current best practice knowledge and skills to Clinical and Care teams.
- Lead and manage opportunities for professional development and education in collaboration with similar roles across HammondCare.
- Grow HammondCare’s capacity to provide complex care, dementia care and palliative and end of life care in the home care setting.

**To be successful in this role, your will have**:

- RN qualified and current registration with AHPRA
- Advanced nursing practice in community and aged care settings, including comprehensive clinical assessment and care.
- Experience in education, advisory, consultative roles.
- Thorough knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards is beneficial
- Demonstrated skill in implementing evidence-based clinical care and leading quality improvement.
- Excellent communication, mentoring, and teaching skills.
- Excellent administrative and computer skills, including digital health and learning records, and data analysis.
- Empathy, patience, and a passion for delivering high-quality, person-centred care.
