Lutheran Services is seeking a compassionate and experienced **Case Manager** to join the team, delivering trauma-informed, culturally responsive support to women and children who have experienced domestic and family violence.**

**About The Role**:
A skilled professional who can provide client-centred case management and also support other case managers through mentoring and modelling best practice.
As the Case Manager, you will:

- Provide specialist outreach case management services to women and children who have experienced domestic and family violence and are exiting refuge
- Develop, implement and review client-driven support and safety plans
- Conduct comprehensive risk assessments, develop individual support plans, coordinating services, and advocating for clients to promote housing stability through tenancy support.
- Model best-practice in trauma-informed care
- Work with external agencies and stakeholders, including housing, legal, and support services
- Lead a specialist portfolio to enhance program capability and service delivery
- Uphold high-quality record keeping, service reporting, and legislative compliance**

**About You**:

- Relevant tertiary qualifications in social work, human services, psychology, or similar
- High-level understanding of domestic and family violence, including CALD and child safety perspectives
- Experience delivering complex case management in a community services setting
- Strong interpersonal skills and the ability to lead through influence and support
- Organised, proactive and values-aligned
- Valid Working with Children Check (Blue Card)
- Current Queensland Driver’s Licence
- Willingness to participate in the on-call roster
