Community Support Incorporated is a leading provider of home care services in the disability and aged care sectors, as well as support coordination. We have been providing services in the community for over 30 years across South Australia and provide a professional and collaborative work environment with a clear focus on person-centred care for our clients.

**About you and the role**
This support is time limited and focuses on addressing barriers and reducing complexity in the support environment, while assisting the participant to connect with supports and build capacity and resilience. The Support Coordinator will bring solid experience in the community services and mental health environment, building collaborative relationships with clients, families, stakeholders, providers, and other organisations. This is a full time, permanent role, and is located at Wayville.
**Key experience required for the role**:
- Strong understanding of the DSOA program
- Accept referrals and engage NDIA/COS Participants
- Engaging with participants and collaboratively implement NDIS plans
- Management of budgets and reporting
- Management of multiple/complex supports from external providers
- Provide support coordination and specialist support coordination to address high level risks
- Bachelor of Social Work, Psychology, Occupational Therapy, Allied Health, Developmental Education or equivalent.
- Experience within the disability and psychosocial disability sector or similar role.
- Strong understanding of NDIS and support planning
- Person-centred problem-solving, decision making and leadership
