Bene Aged Care is a leading provider of culturally responsive aged care services, committed to enhancing the well-being, independence, and quality of life of older South Australians. We are seeking a dedicated and experienced Case Manager - Home Care to join our Community Services team.

In this role, you will play a pivotal part in developing and managing individualized home care packages, ensuring clients receive tailored, high-quality services that align with their needs and goals. Your expertise in case management, assessment, and stakeholder engagement will enable you to provide proactive support to older adults, empowering them to continue living safely and independently at home.

**Key Responsibilities**:
Conduct comprehensive client assessments and develop personalized care plans that align with well-being, reablement, and rehabilitation principles
Coordinate and manage care services within budgetary and funding guidelines, ensuring optimal support for each client
Monitor and respond proactively to clients’ changing needs, implementing early interventions to support health, safety, and social well-being
Facilitate referrals to allied health and community services, ensuring seamless access to appropriate support
Provide leadership and guidance to care staff, ensuring high-quality service delivery and best-practice standards
Engage with clients, families, healthcare professionals, and community organisations to build strong, collaborative relationships
Maintain accurate documentation and reporting, ensuring compliance with aged care legislation, funding requirements, and organisational policies

**About You**:
Degree in Social Work, Allied Health, Mental Health, Nursing, or Disability Services
Proven experience in case management, aged care, or community health services
Strong knowledge of My Aged Care, aged care funding models, and care coordination frameworks
High-level problem-solving, communication, and negotiation skills
Ability to work autonomously and collaboratively in a multidisciplinary team
Proficiency in digital case management systems and strong attention to detail in documentation
Current Driver’s License and National Police Clearance (or willingness to obtain)

Fluency in Italian or experience working with Culturally and Linguistically Diverse (CALD) communities will be highly regarded.
