About you
- Demonstrated contemporary expert nursing clinical knowledge, expertise and skills in nursing practice and leadership enabling the delivery and supporting high quality care provision in chronic conditions nursing to the level required within mandatory qualifications section.
- Demonstrates the ability to build and maintain strong relationships, open communication and a positive environment within a multidisciplinary team, actively contributing to the learning and development of others to a high standard.
- Demonstrates the ability to educate, inform, counsel and support patients and families with contemporary clinical information at an advanced level relevant to patient's care and understanding.
- Applied knowledge of quality improvement frameworks and associated quality processes inclusive of the integration of nursing research, identifying experiences that evidence a professional commitment to contributing to quality improvement.
- Demonstrated ability to interact professionally and maintain professional relationships with all members of the healthcare team both internally and externally to the health service.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
