**Role Overview**:

As a Disability Support Worker at Hope and Care, you will play an essential part in supporting individuals with disabilities, enabling them to lead more independent lives. You’ll help clients with daily tasks, engage them in the community, and provide the care and companionship they need to thrive.

**Key Responsibilities**:

- Provide one-on-one support to individuals with disabilities, assisting them with daily living activities such as personal care, meal preparation, and mobility support.
- Develop and implement individualized care plans that are tailored to each client’s needs and aspirations.
- Encourage and support clients to engage in social, recreational, and community-based activities that foster independence and personal growth.
- Build strong, trusting relationships with clients, offering both emotional support and practical assistance.
- Ensure the safety and well-being of clients by monitoring their health and maintaining a safe environment.
- Maintain accurate and up-to-date records of care provided, client progress, and any significant changes in their needs.
- Support clients with transportation to appointments, social outings, or community activities (driving required).

**What We’re Looking For**:

- A genuine passion for supporting individuals with disabilities and enhancing their quality of life.
- Strong communication skills and the ability to build positive relationships with clients.
- Compassion, patience, and attention to detail in providing care and support.
- Ability to work both independently and as part of a collaborative team.
- Prior experience in disability support is a plus, but not required.
- **Driver’s License**: A valid driver’s license is required to assist with client transportation.
- A current First Aid/CPR certification (or willingness to obtain one).
- **Police Check**: A police check will be conducted for all applicants.

- NDIS Worker Screening Check (required)

- Driver Licence (required)
