THE ROLE
- **Case Management for Homeless Youth**: The role involves providing comprehensive case management support to young people aged 16-24 who are homeless or at risk of homelessness. This includes assessing their needs, developing personalized support plans, and connecting them with
appropriate resources and services.
**Collaboration with Team**: The role requires working closely with the Youth Housing Supports team to
ensure that young people receive the support they need to achieve positive outcomes. This involves sharing information, coordinating services, and advocating for the needs of young people.
