Case Management for Homeless Youth: The role involves providing comprehensive case management support to young people aged 16-24 who are homeless or at risk of homelessness. This includes assessing their needs, developing personalized support plans, and connecting them with appropriate resources and services.
Program Delivery : The successful candidate will be responsible for delivering a range of support services within two programs: Creating Connections and Transitional Support. These programs aim to help young people achieve suitable housing outcomes and enhance their life and living skills.
Collaboration with Team: The role requires working closely with the Youth Housing Supports team to ensure that young people receive the support they need to achieve positive outcomes. This involves sharing information, coordinating services, and advocating for the needs of young people.
Adherence to Guidelines and Policies: The successful candidate must be knowledgeable about and adhere to program guidelines, organisational policies, and procedures. This ensures that all support provided is consistent, ethical, and effective.
Youth-Focused Approach: The role demands a commitment to working with young people in a supportive and empowering way. The successful candidate will need to build rapport with young people, listen to their needs, and help them develop the skills and confidence they need to succeed.
The Ideal Candidate will bring:
- Empathy and Understanding: The candidate should possess a deep understanding of the challenges faced by young people in housing situations. Empathy is crucial for building rapport, providing effective support, and advocating for their needs.
- Strong Communication Skills: Effective communication is essential for building relationships with young people, collaborating with partner agencies, and advocating for their needs. The candidate should be able to communicate clearly, actively listen, and adapt their communication style to different individuals and situations.
- Organisational and Time Management Skills : The role requires managing multiple cases, coordinating with various agencies, and maintaining accurate records. A strong ability to organise tasks, manage time effectively, and meet deadlines is crucial for success in this position.
- Valid Victorian Driver’s Licence |Working with Children Check |Police Check
- Minimum of Diploma in Community Services and experience appropriate to the field (refer to Position Description).
