**JOB TITLE**:Team Leader

**JOB DESCRIPTION**:
To facilitate the team to carry out agreed quality and service outcomes aimed at supporting and assisting persons with a disability in their living and/or community environment.

**TYPE OF EMPLOYMENT**:Part-Time Permanent

**REPORT TO**: Area Managers/Operations Director

**LOCATION**: Toowoomba QLD

**MAIN DUTIES / RESPONSIBILITIES**:

- Ensure the service is managed in alignment with Hopesway’s vision, mission, and values.
- Coordinate and carry out in a cooperative, respectful and flexible manner all agreed tasks and activities aimed at meeting the participant’s personal/living/community needs, team objectives and program goals.
- Ensure safe work practices by following workplace health and safety systems, reporting incidents and taking appropriate actions for hazards, accidents and incidents.
- Accurately complete all relevant Hopesway forms (e.g., incident forms, disturbed sleep, monthly progress report) in a timely manner and forward to managers as required.
- Actively support and create an environment that promotes open communication, continuous improvement, training and skills development.
- Coordinate a team of staff to fill all agreed rostered hours, ensuring that staff attend all mandatory training(s) and are provided with regular feedback on their performance and participate in the annual performance appraisal process.
- Support and assist Disability Support Workers in their daily interaction with participants as required.
- Be involved in on-the-job training and/or service facility orientation of staff, students, trainees and/or volunteers as requested by Managers.
- Facilitate a regular staff meeting (minimum monthly), record outcomes in the minutes and put into place any actions required.
- Demonstrate strong customer service standards and create ongoing relationships with families, clients and other organizations.
- Ensure regular and timely communication with families/carers as required.

**QUALIFICATIONS / REQUIREMENTS**:

- Minimum of Cert III in Individual Support, Mental
- Health, Aged Care, Community Services, Disability work or similar.
- Current WWCC, Blue/Yellow Card, Police Check and First Aid/CPR
- Driver’s License
- Reliable and comprehensively insured vehicle
- Smart phone

**EXPERIENCE**:

- Minimum 6 months relevant experience
- Experience supporting people with a disability
- Demonstrated continuing professional development
- Knowledge of continuous quality improvement principles
- Experience in completing admin tasks, and carrying out duties related to team development

**SKILLS**:

- Excellent verbal and written communication skills
- Ability to build rapport and trust with participants
- Strong time management and ability to prioritize
- Ability to use Microsoft Office suite of programs and other computer systems for record keeping
- Excellent problem solving and conflict management skills
- Ability to work as part of and lead a team and maintain good working relationships

**KEY PERFORMNCE INDICATORS**

**Internal Efficiency**. To ensure that we provide the most responsive and efficient service to our participants within an individualised funding framework
- Ensure effective communication channels with other staff, supervisor, management, participants, carers and other relevant stakeholders
- Progress and shift notes are written following each occasion of support
- Previous shift notes are read at the commencement of each shift to check participant movements/activities and check duties for that shift for the team
- Incidents are reported with completed documentation provided by the end of the first working day following the incident
- All business activities comply with the NDIS Code of Conduct, regulatory and legislative requirements and ethical practices
- Policies and procedures are adhered to
- Ensure all team members display honesty and truthfulness,
- Confronts problems quickly and displays strong commitment to organisational success and inspires others to make and achieve goals
- All shift records are completed in accordance with established documentation protocols

**Participant Outcomes/Service Delivery**: To provide and support your team to deliver quality, person centred support, which promotes the independence, choice and inclusion of people with disability
- Ensure a person-centred approach in service delivery is adopted - Individual program plans are implemented to enable learning, independence and safe living skills - Outcomes achieved demonstrate informed choice by participant and this is documented
- Participant files contain community opportunities that were accessed or supports provided to expand their social networks
- Participant files are maintained in accordance with legislation and organisational requirements - All participants treated with respect and cultural
