Claremont, Australia New Dawn Healthcare Services Full time

MUST HAVE: experience working with mental disability including dementia, anxiety, or autism, acquired brain injury or schizophrenia.
- Provide assistance with home personal care including: showering; grooming; toileting; continence aids and meal preparation and making sure that are dressed well to go out for shopping or other activities
- Provide support with mobility including standing by to assist if required, transferring in and out of bed/chair/car and hoist transfers
- Support service users to manage their medications by prompting or administrating medications if competent, and following medication policy and procedures
- Provide Domestic assistance in the service user’s home including: cleaning, vacuuming, mopping, dusting, re-organising, tidying, laundry, linen changes and unaccompanied shopping as directed
- Provide Social Support including: playing games, talking about area of interest, taking the service user shopping or to local areas for social outing
- Promote independence and ensure person centred care is fostered by offering choice and control to the service user whilst in their home
- Observe, document and accurately report to the Manager any matters regarding the consumers physical, mental and social wellbeing
- Ensure individual rights of consumers and primary carers are protected and respected.
- Ensure incidents or complaints are reported immediately and follow the instructions of the supervisor
- Document any hazards or changes to usual care in the service user’s home file.
- Ensure safe working environments are provided for employees, volunteers, contractors and visitors
- Model compliance with relevant legislation, professional standards, policies, procedures and instructions
- Ensure that all risk, hazards and incidents are investigated, and adequate controls applied
- Consult with team members on safety and health matters
- Refer to each Service users’ individual support plan to follow actions.
- Comply with the Employee handbook
- Assist our clients with medication administration as directed by your Supervisor
- Provide companionship and support during daily activities
- Work with a team of health professionals, family, friends and carers to implement a program of support

**Job Type**: Casual

Pay: $31.76 - $41.76 per hour
