The purpose of this role is to be the first point of contact for HomeCare client enquiries and Personal Care - Community workers (PCC) who are responsible for the delivery of home care services.

The Intake Coordinator plays a key role in the delivery of high-quality community-based care services ensuring service systems are maintained.

**The applicant is also responsible for**:

- Responding to client enquiries and referrals from My Aged Care and Veterans Home Care
- Contacting clients for commencement of services
- Coordinating referrals, service agreements and administration processes to commence service delivery
- Delivering hours within budget and targets
- Follow up with clients for satisfaction surveys of service delivery
- Ad hoc administrate tasks to support the HomeCare team
- Essential Skill/Competencies for the role

**The skills and experience that you will bring to the role are**
- Experience in CHSP and/ or VHC in a similar role
- Assessment and review of clients
- Development of care plans
- Familiar with and understand requirements in the CHSP manual (e.g. wellness and reablement) and DVA guidelines
- Advanced skill in relevant software/ programs - MAC, DEX, Carelink+, Excel, Outlook, Word
- Professional phone manner
- Excellence in customer service
- Demonstrated success in the use of marketing and sales strategies
- Demonstrated ability to create and submit a variety of reports to management
- Able to self-initiate successful actions to ensure the program meets client’s needs and KPI’s (e.g. DEX reviewed regularly and maintained error-free)
