The Support Coordinator is a key member of the team providing Support Coordination for eligible participants. The Support Coordinator is responsible for ensuring participants funds are being spent wisely and that they are connected with appropriate services as part of their NDIS plan. **About The Role**: As a Support Coordinator, you will support our customers to achieve their personal goals and to live satisfying lives in our community. Our approach is simple, we focus on the customer’s choice of supports to achieve their individual goals and ensure they are delivered. **Role Responsibilities**: 1. Person centered assessment and care planning: - In consultation with clients, carers and families facilitate service planning and review - Develop and monitor NDIS Service Agreements and plan budgets with regular reviews/case conferencing - Manage any identified risks within principles of Positive Approach to Challenging Behaviours and Duty of Care, Dignity of Risk - Work within legislated NDIS frameworks 2. Client and stakeholder engagement: - Make relevant referrals, broker services and deal with suppliers as per NDIS plan - Develop and maintain relationships with community organisations to ensure a holistic approach - Develop and maintain relationships with NDIS representatives to ensure high quality communication and service is provided - Ongoing communication with participants, carers/families to ensure they are aware of rights and responsibilities to make informed choices. 3. Organisational Responsibility: - Be aware of and comply with legislation and organisational requirements relating to Workplace Health and Safety - Contribute to quality processes such as management of participant feedback - Actively participate in team meetings - Actively participate in professional development activities (internally and externally) to maintain contemporary knowledge and skills as well as acquire new knowledge and skills **Benefits**: - Interesting and varied caseload, including pediatrics & adult cases - Be part of a small, respectful, and growing team - Great team environment **About You**: You will be able to clearly articulate and demonstrate: - Mental Health, Disability or Community Services qualification or relevant experience (desirable) - Ability to work in a team and individually - Ability to build rapport with a diverse range of customers - Experience working with customers with complex support needs - Driven, energetic and committed - Strong communication skills - Current driver's licence (essential) - Working with Children check and current police check (essential) - NDIS Worker Screen (essential) - COVID-19 Vaccination Certificate or provide a medical exemption evidenced by an authorised medical practitioner This is an exciting opportunity to have a real impact on our customers with a disability.
