Disability Support Worker

Care Central Pty Ltd is looking for Disability Support Workers for its various SDA / SIL homes in South East Suburbs of Melbourne
As a Disability Support Worker, you will provide assistance to a person with a disability with everyday tasks that a person without a disability would be doing for themselves and may include, and not be limited to, the following activities:
PERSONAL CARE- Manual Handling; Showering; Toileting; Grooming; Excercise, communication, personal development
MEAL ASSISTANCE - Meal preparation or assist clients in making their own meals.
COMMUNITY ACCESS - Recreational and social activities includes shopping and accompanying clients to doctor's appointment
HOUSEKEEPING - doing domestic responsibilities such as housekeeping and sanitation.
ADMIN MEDICATION - assisting clients on their medication.
Disability Support Workers will record case notes that detail progress against PCP goals following each shift.
Qualifications:
Roles and responsibilities
The Support Worker’s responsibilities may include:
1. Comprehensive knowledge of the NDIS support items under the NDIS Guidelines.
2. Direct involvement, assessment and support of clients ensuring Fellowship policies and procedures are adhered to.
3. Develop and maintain positive working relationships and linkages with community agencies, clinical services and all other relevant providers to ensure effective communication, referrals and care coordination can occur.
4. Working one-on-one or in a group setting supporting clients to make their own decisions, overcome barriers and strive towards achieving their goals.
5. Advocate on behalf of clients to access services within the community.
6. Monitoring and reporting including monitoring participant referrals, progress against NDIS Goal Plans, data collection and reporting where required.
7. Mandatory administration tasks including data entry into the client management system.
8. Undertake ongoing professional development as required.
9. In addition to the above responsibilities, you are required to also perform any other reasonable duties as directed from time to time.

Training and supervision arrangements
We will provide you with the specific required trainings.
Training plans will be developed and delivered by the CEO/Director.

**Salary**: $28.00 - $48.00 per hour
Expected hours: 20 - 40 per week
