**Our Opportunity to you**
The Community Connect Specialist plays a crucial role in effectively deploying clinical workers to clients in our community.
This **hybrid** role will be working out of our **Aspley office in Brisbane**and from home on a** permanent full time or part time basis.**
**The day to day**
- Providing quality administrative support to the allied health and nursing clinical teams, including reporting, document processing and client data management.
- Supporting the clinical workforce through referral processing and allocation.
- Responsible for document management, customer and referrer liaison, scheduling support of allied health and nursing services, and ongoing client case support throughout our customer’s journey.
- Management and collaboration with new and existing service providers to deliver care to our clients in their home.

**About you**
- Self-motivated medical and office administrator, skilled in customer service
- Strong administration and time management skills, comfortable with using multiple systems
- Experience in service delivery allocation and scheduling, along with effective case management and support skills
- Effective communication skills and building rapport with external stakeholders
- Adaptable and solution focused with the tenacity to overcome challenges
