Pearl Home Care Liverpool-Wollondilly is seeking a **proactive, compassionate, and experienced Care Coordinator / Scheduler** to join our dedicated and growing home care team. This full-time role is pivotal in ensuring smooth, client-centred delivery of Home Care Package (HCP) and NDIS services across the Liverpool-Wollondilly region.
You’ll be responsible for **managing staff scheduling** and also **supporting clients directly**, ensuring their needs are met with professionalism, empathy, and efficiency. **Key Responsibilities**
**Scheduling & Rostering**
- Create and maintain efficient rosters using care management platforms (e.g., VisualCare, ShiftCare, AlayaCare).
- Ensure compliance with SCHADS Award, client preferences, safety standards, and service agreements.
- Coordinate staff placement to prioritise continuity of care, location, client compatibility, and worker qualifications.
- Respond to shift changes, emergencies, and last-minute cancellations with agility and professionalism.
- Actively reduce unallocated shifts and overtime.
**Client Engagement & Care Coordination**
- Act as a key point of contact for clients and families, responding to queries and updating service plans where required.
- Liaise with clients to understand their goals, preferences, and feedback.
- Coordinate care adjustments with Care Managers, Support Workers, and relevant stakeholders.
- Build trusted relationships with clients to promote satisfaction, engagement, and continuity of care.
**Support Worker Liaison & Onboarding**
- Manage regular communications with support workers around availability and shift updates.
- Assist with onboarding and orientation of new staff, including shadow shifts and introductory briefings.
- Match support workers with suitable clients in collaboration with the broader care team.
**Administrative & Office Support**
- Maintain up-to-date, accurate records for clients and staff in accordance with privacy standards.
- Assist with incident reporting, service bookings, and general office administration.
**Essential Skills & Attributes**
- Prior experience in scheduling/rostering in aged care, disability, or community health.
- Strong understanding of SCHADS Award and community care compliance.
- Warm and clear communication skills, both written and verbal.
- Exceptional time management and ability to juggle competing priorities.
- High attention to detail with strong problem-solving initiative.
- Client-first mindset with empathy and professionalism.
- Confidence in Microsoft Office (Word, Excel, Outlook) and care management systems.
**Essential Requirements**
- Previous experience in a similar client-facing coordination or scheduling role.
- Understanding of Home Care Packages and/or NDIS.
- National Police Check (or willing to obtain).
- Ability to maintain confidentiality and professional boundaries.
