As Transition Lead, you will be reporting to the General Manager and joining an office team of 40 and a wider team of support workers of 150+ and rapidly growing

Your role will be project management focussed and therefore you will be working closely with various internal and external stakeholders, including internally Doctors in the leadership team, Nurses from the clinical team, the Risk team and the Operations Manager. Externally you will be liaising with clients, their families &/guardians, Support Coordinators, Allied Health, Doctors & Nurses from hospitals and other key stakeholders.

You will also have the support & exposure of a very dedicated and diverse board with strong experience across the disability & medical sector.

**Role**

This is a new role, created due to the need for a key person to transition clients from external settings from initial contract to their new home.

Your role as Transition Lead will be Project Management led, with the key objective being to provide a seamless transition for clients from settings such as hospitals.

You will be the first point of contact and will be coordinating all areas of the transition to make everything runs on time and to a high quality.

You will be working in a multi-disciplinary team and although not directly managing any employees, you will be managing project expectations and communicating this to all stakeholders to ensure objectives are met.

You will also be managing a pipeline of clients to meet the target numbers of clients to transition over.

**_Your role will include:_**
- Leading regular key meetings (virtual) (weekly & where needed more regularly) to manage teams and project tracking of target transition dates - identifying issues & resolving obstacles
- Coordinating client profiles, intake assessments, financial assessments, client quotes & service agreements, property sourcing and training of the support teams
- Ensuring accommodation is ready for clients through managing the expectations of Area Supervisors responsible
- Reporting on weekly transition planning & pipeline meetings
- Drive continuous improvement

**About You**
- You are from an allied health or clinical background
- You have project management skills and have perhaps used project management tools such as Trello
- You are knowledgeable about the NDIS
- You are collaborative and love coordinating between and liaising with multidisciplinary teams as well as having the ability to be assertive & firm where needed
- You are passionate about supporting people with a disability
