**Sasac Property Services Pty Ltd** is a registered NDIS provider committed to empowering individuals with disabilities to lead independent and meaningful lives. We are currently seeking a dedicated and experienced **Community Worker** to join our team based in **Hoddles Creek, Victoria**.

**About the Role**

The **Community Worker** will be responsible for delivering **specialist support coordination services** to NDIS participants, focusing on building their **capacity, resilience**, and **connection to community and mainstream services**. The role will involve **proactive engagement during times of crisis**, linking clients with appropriate services, and developing support and action plans that facilitate long-term outcomes.

**Key Responsibilities**
- Provide specialist support coordination in a complex service delivery environment.
- Assist participants in understanding and utilising their NDIS plan funding.
- Coordinate services and liaise with stakeholders, including government and community organisations.
- Resolve crisis situations and support development of participant resilience.
- Coach and mentor participants through their NDIS journey.
- Assist with intake processes, rostering, and service planning.
- Monitor outcomes and prepare reports for NDIS plan reviews.

**Qualifications and Experience**
- Diploma or higher qualification in Community Services, Social Work, Disability or related field (AQF Level 5 or above).
- Minimum 1 year experience in a community, disability, or case management role.
- Strong knowledge of the NDIS and local support networks.
- Excellent problem-solving, communication, and project coordination skills.
- Valid Working with Children Check and Police Clearance.
- Current driver's licence.

Pay: $77,000.00 per year

Work Location: In person
