As a Support Coordinator in the Choices team, you will safely transition individuals from the care of the discharging service, ascertain the assistance, intervention and services they would most benefit from, provide emergency interventions where appropriate, then connect them to relevant community services and provide assertive follow up of individuals to ensure they are utilising services they have been connected with.

- A salary of level 4 of the SCHADS award $77,826 - $83,754 per annum.

**Who are we looking for?**:
It is expected that the successful applicant will be able to demonstrate the following:
- Relevant qualifications in a related discipline at degree, diploma, or certificate level/less formal qualification with work.
- Personal goal planning skills that contribute to the achievement of client goals in line with the Model of care.
- Skills in reflective practice and highly developed report writing using analytical reasoning.
- Ability to support and mentor support workers.
- Advanced computer skills, proficient in Microsoft Office and client reporting system.
- Ability to engage and maintain professional relationships with clients and their wider natural supports with multiple needs and issues.
- Demonstrated understanding of complex issues facing people who experience chaotic life circumstances such as homelessness, AOD use, family/domestic violence and mental health issues.
- Willingness and ability to work within and contribute to the vision, mission, core values and the guiding principles of the organization.
- Knowledge and understanding of diversity, gender and social inclusion, particularly Aboriginal culture.
