Position Overview: As a Support Worker for Aged Care and NDIS in the Community, you will play a crucial role in providing essential support to elderly individuals and people with disabilities living in the community. Your responsibilities will include assisting clients with their day-to-day activities, promoting independence, and ensuring their physical, emotional, and social well-being.

**Responsibilities**:

- Provide person-centered care and support to elderly individuals and individuals with disabilities in their own homes or community settings.
- Assist clients with daily living activities, including personal care, meal preparation, medication administration, and mobility assistance.
- Foster a safe and supportive environment for clients, promoting their independence and autonomy.
- Develop and implement individualized care plans in collaboration with clients, their families, and healthcare professionals.
- Support clients in accessing community resources, social activities, and appointments, ensuring their participation and inclusion.
- Monitor and report any changes in clients' health or well-being to the appropriate medical professionals or supervisors.
- Maintain accurate and timely documentation of client interactions, progress, and any incidents or concerns.
- Collaborate effectively with other team members, including healthcare professionals and family members, to ensure coordinated care.
- Adhere to all relevant policies, procedures, and regulations in the provision of care and support services.

Qualifications and Skills:

- Previous experience as a support worker or in a similar role, preferably in aged care and/or disability support.
- Certificate III or IV in Individual Support, Aged Care, Disability, or a related field (or willingness to obtain).
- A genuine passion for providing care and support to elderly individuals and individuals with disabilities.
- Excellent interpersonal and communication skills, with the ability to build rapport and establish trusting relationships.
- Empathy, patience, and understanding when working with diverse clients and their specific needs.
- Ability to work independently and adapt to changing situations while maintaining a high level of professionalism.
- Knowledge and understanding of person-centered care principles and practices.
- Current First Aid and CPR certifications (or willingness to obtain).
- Valid driver's license and access to a reliable vehicle (preferred).
- National Police Check and Working with Children Check (or willingness to obtain).

**Job Types**: Part-time, Casual
Part-time hours: 15 per week

**Salary**: $30.00 - $35.00 per hour

- Armadale, WA: Reliably commute or planning to relocate before starting work (preferred)

Licence/Certification:

- First Aid Certification (preferred)

- Driver Licence (preferred)
