Maitland, Australia Transitional Therapy Full time

We are seeking a compassionate and motivated **Transitional Therapy Support Worker** to join our team. This role involves supporting individuals as they move between care environments—such as from hospital to home or from institutional to community-based settings. You will play a key part in helping clients adjust, maintain continuity of care, and achieve their recovery and independence goals.

**Key Responsibilities**:

- Provide hands-on support to clients during periods of transition, ensuring safety, emotional support, and empowerment.
- Work alongside therapists, nurses, and care coordinators to deliver person-centered support plans.
- Assist with daily living activities, including self-care, medication prompts, and mobility support.
- Help clients develop coping strategies, build routines, and access community resources.
- Monitor client wellbeing, promptly report concerns, and document progress accurately.
- Support clients in attending therapy sessions, appointments, and social/community activities.
- Foster independence while respecting individual rights, preferences, and cultural values.
- Promote continuity of care by liaising with other services, family members, and care teams.

**Requirements**:

- Experience in a support worker, care assistant, or mental health role (transitional care experience is a plus).
- Understanding of recovery-based approaches, trauma-informed care, and safeguarding principles.
- Excellent communication, problem-solving, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Flexibility to work various shifts, including evenings or weekends, if required.
- Relevant qualification in Health and Social Care (C4) or equivalent preferred).

**Desirable**:

- Experience supporting clients with mental health, substance misuse, or complex needs.
- Training in de-escalation techniques, first aid, or therapeutic approaches.
- Driving license and access to a vehicle (depending on location and outreach responsibilities).

Pay: $35.00 - $45.00 per hour

Expected hours: 15 per week
