We are growing in leaps and bounds and therefore have a requirement to fill 02 Service Leader roles (PPT x 1 and FT x 1). These roles will be located at our office in Balwyn, Victoria.

You will be responsible for supporting the Service Manager in delivering, scheduling, rostering programs and supports for people living with a disability, to ensure the delivery of high quality, responsive and sustainable supports that meet or exceed participants’ needs and budget requirements. You will work closely with our operational teams to ensure cohesive, aligned and seamless service delivery.

**Key Responsibilities**:

- Comply with the Multicap code of conduct, policies and procedures at all times
- Commit to our principle of participant-centric support and work within the NDIS Service Standards framework at all times
- Identify, record and report all incidents, injuries, near misses and hazards
- Exercise Duty of Care at all times
- Lead and support the DSWs delivering supports, providing mentoring and guidance to ensure that all supports delivered consider and align with participant goals
- Support the DSWs to ensure they’re effectively undertaking case notes on a daily basis and that they’re maintained appropriately in the CRM, actioning and following up with stakeholders as required
- Ensure DSWs maintain current knowledge of their participants’ records and support/medication needs
- Oversee the development of personal plans (IPPs, IEPs, IRPs etc.) by DSWs in collaboration with the participant/families and ensure plans are updated regularly and reviewed annually
- Ensure participant details are accurate and updated as required
- Monitor and respond to ZenDesk tickets enquiries from families (knowledge of ZenDesk system desirable)

- Previous experience within the disability sector and/or NDIS environment, providing support to participants and families living with disability (Highly regarded/Desirable)
- Work with a fantastic team and support the Service Manager to help grow the business and deliver wider supports

**Job Types**: Full-time, Part-time
Part-time hours: 30 per week

**Salary**: $56,889.00 - $60,050.00 per year
