**About the role**:
We are looking for an experienced and enthusiastic Case Manager role based at the Auspire Care office, Coburg VIC 3058 to manage a caseload of elderly clients.

**Conditions of Employment**
- Appointment will be in accordance with the Social, Community, Home Care and Disability Services Industry Award, Level 4 PPT 1.
- There is a three-month probationary period for new employees.
- Auspire Care is an equal-opportunity employer.

**Key Responsibilities**
- Development and implementation of high-quality individual care plans
- Development of supportive relationships with professional staff, suppliers, and families to ensure positive outcomes.
- Ongoing communication with clients and family members to foster partnerships and ensure choice and control of their individual care plans.
- Compliance with internal policies and procedures, statutory and contractual requirements in relation to the management of service delivery

**Skills & Experience**
- Diploma or equivalent qualification in Community Services
- Demonstrated experience in Case managing.
- Registered Nurse - desirable
- Experience in implementing individual care plans, health, and well-being clients.
- A **criteria** Use examples where appropriate and do not assume that we will know you can do something.
- A covering education, work history, specialist skills or personal qualities along with contact numbers for you and who can advise the panel on your relevant skills and experience.
