Our Home Care is currently searching for a talented, committed and experienced**,** **Clinical Care Manager** to join the BASScare team.

**Job Role**

We have an exciting opportunity for a skilled and driven Clinical Care Manager to join our growing Home Care team. The Clinical Care Manager is responsible for delivering outstanding personal support to our clients, enabling them to live life with independence, choice, and dignity.

**Key Responsibilities**
- Act as the primary contact and advocate for Home Care clients, developing and monitoring person-centred care plans in partnership with Consumers
- Conduct comprehensive assessments to support Home Care Package (HCP) clients, ensuring care plans align with their clinical needs and best practices.
- Provide clinical oversight and guidance to Care Managers, offering expert advice on clinical matters, risk management, and complex cases.
- Deliver urgent clinical care when external nursing services are unavailable.
- Actively participate in Clinical Governance meetings to ensure continuous improvement in clinical practices.
- Develop and maintain effective working relationships with key stakeholders to raise the profile of Home Care Packages within the community.
- Maintain accurate records of assessments, care plans, budgets, and communications, while liaising with referring agencies and service providers to ensure comprehensive client support.
- Ensure compliance with Aged Care Quality Standards, funding agreements, and legal requirements.

**About you**
- Registered Nurse or Enrolled Nurse qualification.
- Industry experience in a similar role, preferably within community aged care or disability settings.
- Proven experience working as part of a multi-disciplinary care team.
- Strong ability to interpret and implement policies and procedures.
- Excellent communication skills, both written and verbal.
- Capacity to work independently.
- Current Police Check, First Aid, and CPR certifications.
- Full driver’s license.

**Hours of work**

Full time, Monday - Friday, 9:00 am - 5:00 pm
