The Role

An opportunity has become available for two Home Care Coordinators to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.

You will

Coordinate client-focused care for clients per safety and quality standards
Collaborate with clients, carers, and professionals for effective care planning
Manage cases through assessments, reviews, and home/community visits
Oversee recruitment, rostering, payroll, training, and support for Home Care Workers

About you

To be successful in this position you will require:
Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
Tuberculosis screening, to identify if the tuberculosis vaccination may be required
Evidence of an unrestricted Open Drivers Licence to operate a government vehicle
Declaration of serious discipline history must be disclosed
Aged Care Check will be required for new employees (GCH will cover cost)

Salary

Permanent full time and Temporary full time (6 months)
Base salary ranging from $94 926 - $100 071 per annum, $3638.50 - $3835.70 per fortnight
