**Description**:
**About the Role**
As a **Personal Care Worker** in the community, you’ll work collaboratively with a team to support older clients in their homes. Our non-medical care model includes services necessary to help patients safely and smoothly transition from a healthcare facility like a hospital to their home. Right at Home work with several partnering organisations to provide staffing solutions:

- Communication with family and healthcare providers
- Regular visits
- Medication reminders
- Transportation to doctor appointments
- Meal preparation
- Housework

**What You’ll Need**
- Certificate III in Aged Care (or equivalent experience)
- Certificate IV in Aged Care (or equivalent experience)
- Current Police Check Clearance
- Valid First Aid and CPR Certificate
- NDIS Paid Worker Screening Check
- NDIS Orientation Module Certificate
- Australian Driver’s Licence
- Comprehensive Car Insurance
- 100 Points of ID (e.g., Medicare Card, Birth Certificate, Passport)

**Vaccination Requirements**
