**The Transition Care Program (TCP) is seeking an experienced, innovative and flexible individual to work as a member of the TCP Case Management team.**
The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.
TCP operates in both the Community as well as two Residential Aged Care environments, offering scope for clinicians to enhance their experience along the care continuum.

What You Will Be Doing
The successful applicant will need to be flexible in terms of working across both residential and community areas. The Case Manager will need to demonstrate sound clinical judgement, assessment and practice skills, and have an understanding of the issues affecting patients with complex psychosocial needs and an interest in aged care issues.
The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team. Further to this, the applicant will need to support case management service provision and clinicians within the TCP program as required.
**What You Need**
We anticipate you will have:
- Eligibility/Membership of relevant professional body
- Experience in a sub-acute and/or community setting
- Demonstrated competence in a specific RN or Allied Health area of practice
- Capacity to supervise student
- Knowledge of / experience in Aged/Residential Care settings.
- Current Driver’s Licence
- Demonstrated participation in Professional Development.
- Interest and/or experience in Quality improvement and research activities
- Computer literate
