Hours: Monday to Friday | 7:00 AM - 3:00 PM (or variation depending on operational needs)

We are seeking a hands-on, caring, and passionate Care Manager (Grade 5 RN) to lead our Memory Support Unit at Homestead Estate in Wallington.

What You’ll Be Doing

As a Care Manager, you will provide exceptional dementia care, working closely with a high-performing team to ensure a safe and supportive environment for residents. Your key responsibilities include:

- Delivering comprehensive, person-centred nursing care
- Developing recommendation plans & behavior support strategies
- Managing medication administration & wound care
- Driving continuous clinical improvement for excellence in care
- Supporting residents, families, and visitors with professionalism
- Ensuring timely assessments, care plans, and reviews
- Leading clinical education & staff competency training
- Fostering a positive and collaborative workplace culture

What Matters to Us

We’re looking for a dedicated professional with:

- Minimum two years’ experience in a clinical leadership role
- Strong understanding of Aged Care Quality Standards
- Division 1 Nursing qualifications & AHPRA registration
- Up-to-date COVID-19 & 2024 influenza vaccinations
- NDIS Check (incl. Police Clearance)
- Eligibility to work in Australia
