$99,015.38 - $107,732.31 (pro rata to be applied) plus Super
An exciting opportunity has arisen for a capable, community-minded professional to pilot a new Aged Care Navigator service in Boroondara. Reporting to the Active Ageing Lead, this is a part-time role (0.8 EFT) working 30.4 hours per week until 30 June 2026.
A little bit about the role
The Aged Care Navigator will be the key point of contact for older people in Boroondara seeking support to access aged care services, stay connected to their community, and age well.
Working across Council sites including libraries, neighbourhood houses, seniors centres and community hubs, the role will provide practical information and person-centred support to older residents and their families.
**Key responsibilities include**:
Supporting people to navigate the My Aged Care system and access local services
Delivering one-on-one consultations and hosting informal drop-in sessions
Advocating for older residents’ needs with service providers and stakeholders
Sharing information and building awareness of the pilot program
Contributing to program evaluation and future planning

You are an empathetic and community-focused professional who is confident working with people from all walks of life, including those with complex needs. You will bring strong interpersonal skills and a practical understanding of aged care systems and supports.
What are we looking for?
**To be successful in this role you will have**:
Qualifications and/or experience in community development, social work, health, or a related field
Experience working with older people or people with complex needs
Strong communication and community engagement skills
Knowledge of (or the ability to quickly learn) aged care systems and supports
A current Working with Children Check
