**Job description**
Sensible Care is an approved provider of Home Care Packages and a registered provider of NDIS Disability Support Services. **About the Role**:
With the expansion of the company, we are looking for a Home Care Packages (HCP) and NDIS Support Coordinator. This role is suitable for a person who enjoys human interactions, building new relationships, managing staff members and passionate for creating a difference in others’ lives. Previous NDIS support coordination experience is highly desirable. Sensible Care will provide training for Home Care Packages care coordination skills.
**Tasks and duties**
- Visiting the clients at their homes and undertaking comprehensive assessments; identifying their goals and needs; producing care plans (for HCP) and the corresponding service schedules
- Setting up supports and services, including helping clients understand service agreements and service bookings
- Regularly reviewing their support and care arrangements and make changes where necessary
- Managing connections with supports and services, verification of charges and liaising with services if charges do not seem correct
- Considering client’s goals, needs and circumstances, matching care and support staff with clients
- Acting as the first point of call for clients, families, other nominated representatives, healthcare professionals and external Support Coordinators
- Working closely with care and support staff members to ensure scheduling is appropriate and handling any issues or queries from the team
- Work with the broader team on quality and continuous improvement initiatives
- Taking an active role in building relationships with client groups and referral sources and representing Sensible Care in business development activities.
- Ensure all documentation for clients are accurately maintained and drafting reports
**Essential key selection criteria**:
- Previous experience and demonstrated knowledge in NDIS support coordination
- Time management skills to effectively manage workload and meet deadlines
- A high level of interpersonal skills including the ability to develop and maintain relationships within the team, with clients and participants, across service providers, and other stakeholders
- An Australian Driver’s Licence and own vehicle
- Strong rostering capabilities and ability to manage competing priorities
- Strong attention to detail and well organised
- A commitment to providing the best quality support and services to our clients
This role also requires the successful applicant to have or obtain an NDIS workers screening check and national police check.
**Job Types**: Full-time, Part-time
Part-time hours: 25-38 per week
**Salary**: $58,374.00 - $99,366.00 per year
