**About the Role**

Grampians Health Ballarat is seeking a motivated and enthusiastic clinician to join our Emergency Department Care Coordination (EDCC) team.

This is a unique opportunity to work in a dynamic and fast-paced environment where you’ll facilitate timely access to an integrated system of community-based services. EDCC staff play a vital role in supporting person-centred care, safe discharge, and service continuity across hospital and community settings.

You will be supported with 2 days of paid supernumerary EDCC training alongside an experienced team member.

**Key Responsibilities**

Facilitate timely and safe discharge from the Emergency Departmen
- t Coordinate referrals to community-based service
- s Provide a primary point of access for external service provider
- s Promote continuity of care between acute and community setting
- s Collaborate effectively with ED staff and the wider multidisciplinary tea
- m Support patients and carers to participate in care planning and decision
- s

**Skills & Experience**

Registered Nurse (Division 1) with AHPRA (desirable
- ) Postgraduate qualification in a relevant area (highly desirable
- ) Demonstrated experience in acute and community car
- e Strong skills in assessment, discharge planning and interdisciplinary teamwor
- k Excellent interpersonal and communication skill
- s Ability to work both independently and collaboratively in a fast-paced settin
- g Willingness to pick up additional EDCC shifts when availabl
