Roselands, Australia Clinical Assessor Home Care Full time

Clinical Assessor Home Care is looking for an experienced and exceptional Support Worker on a Casual basis to join our team and reporting to the Home Care Manager and Case Coordinator.

Role and Responsibility
- Provide support to consumers, across numerous mediums such as personal care, meal preparation, and household chores.
- Provide emotional support and companionship to individuals, ensuring their wellbeing and safety.
- Assist individuals with their medication and attend medical appointments with them as required.
- Deliver a service that meets the needs of consumers while operating in line with legislation, regulation and best practice guidelines.
- Promote independence and encourage individuals to achieve their goals and aspirations.
- Maintain accurate records and document all activities in accordance with company policies and procedures.
- Represent the brand with enthusiasm and pride.
- Plan, prepare and execute business objectives in an aim to assist the community.
- Communicate with relevant internal and external stakeholders involved in the consunmer's care, including family members, social workers, and allied health professionals in an efficient manner.
- Attend regular team meetings and participate in training and development opportunities to enhance your skills and knowledge.

Essential Skills and Qualifications
- At least Certificate III and/ or IV in relevant fields such as Individual Support or Community Services Work
- Excellent communication and interpersonal skills
- Currently hold or willing to obtain a National Police Check
- Empathy, compassion, and a positive attitude towards supporting others
- NDIS Screening Check - not essential but highly desirable
- Ability to maintain confidentiality and respect individuals' privacy
- Knowledge of safeguarding procedures and willingness to follow company policies and procedures
- A valid driving license and access to a vehicle is desired
