
Disability Support Worker Needed with Mental Health

Willingness to work with individuals with complex behaviours of concern.
- Provide individual support services and direct care for people with disability and mental health issues.
- Support clients to participate in age-appropriate activities and tasks, including employment, in-home support and community access.
- Personal Care Profile including (but not limited to): Dressing/grooming, Showering/bathing, Toileting, Preparing meals, Assisting with participant's physical and mobility requirements,
To provide other support to participants as required, such as:
- Personal administration, Shopping for and with Participants, banking and paying bills (where applicable), supervision of participant's medication requirements, washing and ironing personal clothes, and ensuring that bathing areas and participant's appliances are clean.
- Performing household duties and ensuring the house is clean and tidy—tidying participants' rooms. Identifying the need for and supporting the participant to attend medical/therapist appointments
- Transport as required.
- Facilitate community inclusion and participation, dependent on individual needs, abilities, and preferences
- Support participants to establish and maintain relationships and interests through access to community activities, venues, and services.
- Monitor the safety and well-being of the clients and report any concerns to the Manager.
- Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication book as required.
- Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.
- Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.
- Provide backup and support to other staff, as necessary.
- Participate in Performance Development meetings to identify ongoing training and support opportunities.
- Notify the service if you cannot work your rostered shifts with as much notice as possible.
- Contribute to Case reviews with Employment Consultants and Facilitators.
- Take reasonable care for his or her own health and safety
- Take reasonable care that his/her acts or omissions do not adversely affect the health and safety of other persons.
- Report any quality and safety incident or hazards at work to their Manager or Supervisor.
- Carry out their roles and responsibilities as detailed in the relevant policies and processes.

**Salary**: $30.00 - $33.00 per hour

Ability to commute/relocate:
- Tarneit VIC: Reliably commute or planning to relocate before starting work (required)
**Experience**:
- support worker: 3 years (required)
- Mental health counselling: 2 years (preferred)
Licence/Certification:
- First Aid Certification (required)
- Working with Children Check (required)
- National Police Certificate (required)
- Driver Licence (required)
- Full Driver Licence (required)
Work Authorisation:
- Australia (required)
