Full Job Description

This position will see you provide assistance to our Senior Support Coordinator, as well as manage a caseload of NDIS clients, supporting them to identify, coordinate and implement a range of supports to enable greater individual participation in their home and their community.

In addition to the aforementioned, this role will also be responsible for:

- Assist the Senior Support Coordinator with the administration of NDIS plans, implementation of plans and reporting.
- Accurate and timely data entry pertaining to NDIS.
- Provide timely and comprehensive information and advice in response to enquiries from potential and current clients / families regarding NDIS.
- Develop, continuously improve and streamline our internal processes to ensure efficiency, scalability and sustainability.
- Ensure that databases and systems are kept up to date.
- To organise the care team whilst considering the changing needs of the participants.
- ▪ To clearly set out the expectations for each shift and direct the team accordingly.
- ▪ To promote dignity, respect and choice for participants, ensuring that the care team work within this ethos at all times.
- ▪ Ensure that care and support is provided for participants in a timely way; upholding and improving standards of care delivery.
- ▪ **Assessing the service needs of clients and linking them to the appropriate resources and providers.**:

- ▪ Advocate for adequate, timely and cost-effective services
- ▪ Respond to any issues that occur during the delivery of services.
- ▪ Schedule appointments for clients, answer phones and track clients’ service records
- ▪ Monitor the services being provided and stay up to date on any services being introduced or discontinued.
- Experience in NDIS (Support Coordination, Planning, Plan Management) and Disability services, coupled with administrative experience.
- A willingness to learn, to take ownership of work, use Initiative and be resourceful.
- Prepare yearly reports for participants.
- Exemplary customer service experience or demonstrate similar attributes.
- Meticulous attention to detail as well as effective time management, organisation and problem-solving skills.
- Proficient written and verbal communication skills.
- Proficient use and knowledge of all Microsoft programs.
- Proven ability to use interpersonal skills in a team environment. Work collaboratively with a range of stakeholders including peers, employees, external organisations.
- Current Open drivers license.
- Eligibility to clear probity checks including a Disability Worker Screening Clearance Card.
