**The Position**

**St John of God Richmond Hospital **is a private mental health hospital northwest of Sydney providing comprehensive and holistic mental health care services in a tranquil setting. Providing multidisciplinary care to all patients based on an individualised care model, the hospital wards maintain strong links with community mental health services, other primary health care providers and social services to ensure patients receive ongoing care after leaving hospital
- We currently have a rare and exciting opportunity for a
- **Senior Registered Nurse** to join the team as
- **Transition Coordinator.**
- As an integral part of the Admissions Team, the Transitions Coordinator Manages the discharge planning function in conjunction with Nurse Unit Managers (NUM’s) and with reference to optimising patient service and financial objectives. You will work closely with the Manager Consumer Pathways to coordinate admissions and liaison with consumers, carers and referrers seeking to access to care, as well as the timely coordination of admissions processes.

**Key responsibilities include**:

- Work closely with Admissions Corrdinator to ensure even distribution of admissions/discharges
- Actively communicate with other service providers regarding 'at risk' patients, who may require coordination of multiple services
- Work collaboratively with the medical and multi-disciplinary staff, internal and external to the organisation
- Coordinate timely follow up of patients after their inpatient admission and responds appropriately to any clinical concerns that may be raised
- Participates in quality improvement and evaluation processes related to the management of patient care.
- Assist the Admissions Team as required in order to facilitate transition in from the community or from other facilities
- This position will be offered on a permanent full time basis, working 76 hours per fortnight.
- A willingness and capacity to embrace the Mission and Values of St John of God Health Care.Possession of qualification in Nursing with at least 3 years post registration clinical experience in mental health.
- Registration with the Nursing and Midwifery Board of Australia as a Registered Nurse.
- High-level communications and interpersonal skills, with a demonstrated ability to understand and respond to stakeholder needs.
- Exceptional organisational and time management skills with an ability to multi-task.
- Well-developed analytical and problem solving skills.
- Demonstrated ability to work collaboratively in a multidisciplinary health care environment.
- Demonstrated understanding and commitment to quality improvement and the National Safety & Quality Health Service Standards process.
