Monash Health Emergency Department Care Coordinators are an integral part of the dynamic multi-disciplinary team providing care across our three Emergency Departments at Casey Hospital, Dandenong Hospital and Monash Medical Centre. **About the Role**

Are you looking to change work areas and be part of an Interdisciplinary Allied Health team working in Monash Health's fast paced Emergency Departments?

If so, then a fantastic opportunity exists for enthusiastic, motivated and dynamic Allied Health professionals to join the RAPID (Rapid Assessment Planning Intervention and Discharge) Care Coordination teams working within the 3 Monash Health Emergency Departments - Clayton, Dandenong and Casey (Berwick).

**About You**

You are a Physiotherapist, Occupational Therapist or Social Worker. You will possess a strong patient-centred focus, well developed leadership and communication skills and bring a range of clinical experience. We are seeking applicants with excellent problem-solving skills and a positive, can-do attitude. The ability to work autonomously is essential. Experience in Acute Healthcare is essential, and previous experience working in an Emergency Department would be beneficial.
