**About the role**
This is a 3-day (24 hrs/wk), Permanent Part-Time position with a view to Full-Time. There is a requirement to travel between our office based in West Gosford and client homes based across our diocese for assessments and care management.
**Duties**
- Provide effective case management to our clients and be flexible and responsive to ensure we can offer high quality and continued support to clients changing care needs.
- To provide exceptional customer service across all external and internal stakeholders.
- To support a personalised on boarding process of new clients for all our Senior Living (Aged Care) program/s, which includes guiding them on the eligibility process through My Aged Care to ensuring they are provided the best possible care.
- Provide guidance, support, and training to our team to improve our quality of care.
- Collaboration with internal/external stakeholders, including strengthening our relationships within the Aged Care industry, while building referral pathways.
- Build on our positive brand name within the Community, with aim to increase referrals to our services.
**Requirements**:
- Certificate IV in Aged Care, Community Services or related field **OR** equivalent experience/education
- Experience working with Seniors, including those living with Dementia
- Good knowledge of the Aged Care system and services
- Demonstrated experience in client budget monitoring and care management.
- NSW Driver’s Licence and access to an insured vehicle
- Access to personal smart phone
- Current Senior First Aid or willingness to complete this training
- Willingness to complete a Criminal History Check
