- **Supporting Clinical Care Manager**

We are currently seeking a Clinical Care Team Leader to join our team at North Fremantle

**Duties & Responsibilities include**:

- Leading and participating in the delivery of a high standard of person-centered resident care.
- Providing a high standard of ongoing care management and perform nursing interventions.
- Coordination of comprehensive completion of individual holistic assessments and care plans for new and existing residents.
- Ensuring coordination of all clinical documentation is completed accurately and in a timely manner.
- Assisting the Clinical Care Manager with monitoring of medication management.
- This position reports directly to the Clinical Manager

**About you**:

- As a collaborative member of the Clinical leadership team, you are required to have:

- Current AHPRA Registration as a Registered Nurse.
- Knowledge and relevant experience in clinical services in a complex nursing or aged care setting.
- Understanding or ability to learn the attributes of the Commonwealth Funding System in relation to AN-ACC/ACFI.
- Ability to provide direction and expert advice in the treatment of acute care episodes.
- Demonstrated understanding of the relevant Laws, Acts, Regulations, Charters and Codes of Practice.
