**About The Role**

The Care Coordination Administrator is responsible for providing administrative support to enable the efficient delivery of care management and package management services, including care coordination, to Home Care Package recipients.

We are seeking multiple, full-time applicants with customer service experience who will champion our consumer-directed care approach, and provide ongoing support to consumers.

**Duties**
- **Onboarding New Clients in our Customer Relationship Management (CRM).**
- **Support Care Coordinators in onboarding to our Care Coordination Program.**:

- **Data capturing and entry for care budgets and care plans.**:

- **Administrative support and document development to improve resources for our Care Coordinators**:

- **Liaising with clients and/or their representatives on aspects of their Home Care Package.**:

- **Coordinating and performing periodic client service reviews; requests for upgrades and supplements; leave and other related activities.**:

- **Maintaining and updating care management systems relating to our clients and their representatives, stakeholders, service providers and government agencies.**:

- **Acknowledging and actioning requests for consumers from Care coordinators**:

- **Scheduling care plan meetings with consumers, their representatives and coordinators as required**:

- **Provide managed organisations with information required for them to achieve compliance with Trilogy Care's coordination program**

**Salary & Benefits**
- **$55,000 - $65,000 per annum (commensurate with experience) + 10.5% super**:

**Mandatory Requirements**
- **Current Federal Police Clearance (not more than 12 months old) and international Police Check if you have lived overseas in the last 10 years**:

- **Evidence of Australian work rights (Passport, Birth Certificate, etc).
