**We’re currently seeking dedicated Support Workers to provide**_**personal care**_ **services during key times of the day.**

Applicants must be available for the following shift times:

- **Morning shifts: 7:00am - 9:00am**:

- **Evening shifts: 6:00pm - 8:00pm**:

- **Weekend availability is required as part of a rotating roster.**

We’re currently seeking Support Workers to support participants based in the **Newcastle**and **Lake Macquarie**areas.

**Your Responsibilities**
- Foster a supportive relationship with participants and their families
- Value the participants choices make
- Provide a person centered approach that upholds human rights
- Comply with policies, procedures and code of Conduct of Hunter Community Hub, aligning with the organizations mission and values.
- Provide assistance to participants with personal support tasks, including activities of daily living, domestic support, meal preparation, transportation and engagement in social and community activities.
- Carry out administrative tasks including case notes and timesheets
- Maintain Strict confidentiality both internally and externally
- Embrace workplace diversity and foster an inclusive environment
- Welcome constructive feedback
- Communicate promptly with staff when issues arise, while documenting them accordingly

**Essential**
- Current Driver’s License
- Fully registered and insured vehicle
- Working with Children Check
- NDIS Workers Check
- Computer literate
- Satisfactory numeracy and literacy skills

**Desired**
- Certificate IV in Disability or Mental Health
- Experience with Mental Health
- Nursing Experience
