**About the role**

As a female Support Worker at Activ, you will care for customers who have varying needs. For this role, we are seeking an individual experienced with complex high-care needs including behaviours due to a variety of medical conditions. Customers have a wide range of professional supports and full training in their care plans is provided. Please review the rosters before applying;

A typical day in the role may look like
- Delivering a wide range of domestic supports within a customer’s residence;
- Supporting person-centred plans that identify a customer’s life goals;
- Supporting customers to engage in community-based activities;
- Ensuring positive behaviour support plans are followed; and
- Administration duties and mandatory daily reports.
- Well-developed interpersonal skills with the ability to build effective working relationships;
- Strong communication skills;
- Ability to follow person-centred plans and assist people to achieve their goals and outcomes;
- Extensive experience in supporting customers with behavioural support plans;
- Experience in supporting customers with autism and challenging behaviours;
- Willingness to assist with personal care and domestic duties;
- Cert III in Disability or Individualised Support desirable;
- Proven administration and computer literacy skills;
- Valid WA drivers licence and a willingness to use your private vehicle (aligned with Activ’s motor vehicle allowance rebate).
- Police clearance and/or NDIS worker screening check.
- NDIS Worker Orientation Module

**Activ staff working in roles that require regular attendance to our residential disability accommodation homes must be fully vaccinated against COVID-19 and receive a booster dose within one month of becoming eligible.**
