We are seeking experienced, compassionate, and dedicated professionals to coordinate care services for our elderly clients. As a Care Coordinator within ECH, you will have the profound opportunity to enhance client well-being and independence while building trusting relationships with our clients and families.
Our goal is to enable our clients to live confidently and comfortably in their own homes and get more out of life. With the support and guidance of our leadership and clinical teams our Care Coordinators undertake comprehensive assessments and develop care plans for our clients that enable them to reach their individual goals.

We are looking for someone:
- Who is committed and compassionate.
- Who is sensitive to the needs of older people of different social, economic and cultural backgrounds
- Who can build rapport with clients and their families
- Who describes themselves as kind, dynamic and energetic
- Who is ready to empower others and work cohesively with colleagues to deliver the best outcomes for our customers
- Who can adapt to quick change and has an organised approach to managing their caseload
- Who owns continuously learning and seeking opportunities to contribute, innovate and solve problems
